Jeff Cohn, MD, MHCM, is the Medical Director at Common Practice in Philadelphia, PA, USA. This company focuses on the conversations about serious illness and death that are too often avoided, or conducted poorly and painfully.

Dr Cohn graduated with an MD from Jefferson Medical College in Philadelphia in 1980. He then followed an internal medicine residency at Einstein Medical Center in Philadelphia and a fellowship in haematology/medical oncology at Emory Hospital, Atlanta, GA, USA, and Johns Hopkins Hospital, Baltimore, MD, USA. He received a Master’s degree in Health Care Management from Harvard School of Public Health, Boston, MA, USA.

Dr Cohn was Chief Quality Officer and Patient Safety Officer for Einstein Healthcare Network in Philadelphia for 11 years; during this time, he spearheaded the creation of the palliative care programme at Einstein. He was also President and CEO of the Plexus Institute, a non-profit organisation that focuses on improving the health of communities and organisations by using innovative methods concentrating on the sociocultural aspects of change. He took up his current position at Common Practice in July 2015.

Dr Cohn is a long-time change agent and student of behaviour change. He is particularly drawn to solutions such as the positive deviance framework, which helps communities solve complex, intractable problems by identifying and building on what has been shown to work.
